Travel Registered Nurse Home Health Job
Registered Nurse – Home Health is a travel, temporary assignment in Silverdale, Washington. The start date is May 4, 2026, for a 14-week contract with 40 guaranteed hours each week. Shifts are 8 hours. This position involves skilled nursing care in the patient’s place of residence, coordinating care with the interdisciplinary team, patient/family, and referring agencies within a home healthcare setting.

Responsibilities
- Assess home health and/or hospice patients in stable and emergent situations to identify physical, psychosocial, and environmental needs; document findings and report to the team.
- Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
- Gather data on physical, psychological, social, spiritual, and cultural factors that influence patient/family health status and incorporate this into the POC.
- Maintain up-to-date patient records, ensuring visits, contacts, problems, plans, actions, and goals are clearly stated and updated as changes occur.
- Provide care using infection control measures to protect staff and patients (OSHA standards).
- Educate patients and families about care, medications, home safety, and related topics; tailor education to age, condition, culture, and learning readiness; promote ongoing self-care practices.
- Supervise and provide clinical direction to HHA/STNA/LVN staff to ensure quality and continuity of service.
- Assess and reassess pain; implement appropriate pain management strategies and educate patients/families on pain control.
- Arrange referrals to appropriate agencies when needed and monitor cases for third-party payer compliance.
- Participate in on-call responsibilities as assigned and serve as a liaison to patients, families, team members, and physicians.
- Attend interdisciplinary team meetings and contribute to care coordination and outcome improvement.
Requirements
- Current state Registered Nurse license in Washington (Required)
- CPR certification (Required)
- OASIS experience (Required)
- Homecare Homebase software experience (Required)
- 1–2 years of RN clinical care experience (Required)
- 2+ years of home health experience (Required)
- Knowledge of medications and correct administration; venipuncture and IV care; wound care and decubiti management; indwelling catheters and drains care; safe use of durable medical equipment; and ability to develop and initiate bowel/bladder training programs when applicable (as part of core home health nursing skills)
- Graduation from an accredited Associate Degree in Nursing or Nursing Diploma program (Required); BSN (preferred)
- Valid driver’s license with reliable transportation and auto insurance
- Ability to work in a home-based care environment and travel locally as needed
- CHPN certification (Preferred)
