Responsibilities
- Coordinate discharge planning and care transitions across hospital units for diverse patient populations, ensuring timely placement at the next level of care
- Conduct comprehensive needs assessments and develop collaborative plan-of-care with the interdisciplinary team, including Social Services
- Manage a caseload of approximately 25–30 patients, documenting progress and coordinating DME orders, home health, or facility placement as appropriate
- Maintain accurate EPIC charting, participate in rounds, and communicate plan-of-care with patients, families, and care team
- Support across hospital areas and, as needed, a sister campus to ensure seamless care transitions
- Ensure adherence to established policies, patient safety standards, and regulatory requirements; provide education to patients and families about discharge plans

Requirements
- Licensure: Active RN license in Washington or pending license accepted; or MSW where applicable
- Certifications: BLS required
- Experience: Minimum 1 year acute care experience with discharge planning / care coordination
- Skills: Strong acute hospital care knowledge, needs assessment, discharge planning, and care coordination; EPIC charting experience
- Population scope: Ability to work with Neonates through Geriatrics
- Scheduling and mobility: Willingness to float to all hospital areas and to a second campus as needed; weekend coverage as required
- Education: RN (Bachelor’s or higher preferred) or MSW where applicable
- Compliance: Must be able to meet the unit’s workflow and policy requirements; behavior health experience alone does not meet acute care requirements
- Holidays: Willing to work 3 of 5 designated holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year’s Eve, New Year’s Day)
