Your responsibilities as an LPN in Long-Term Care include administering medications, monitoring vital signs, performing basic wound care, assisting with activities of daily living, documenting clinical notes, contributing to care plans, and communicating changes in residents’ conditions to the licensed team. You’ll coordinate with RNs, physicians, social workers, and families to ensure continuity of care that respects residents’ preferences and goals.
