Responsibilities
- Assess residents and develop individualized physical therapy plans of care.
- Implement therapeutic interventions including therapeutic exercise, gait training, balance, and functional mobility activities.
- Reassess progress regularly, adjust plans, and document outcomes.
- Collaborate with an interdisciplinary team to optimize residents’ mobility and quality of life.
- Educate residents and families on home exercise programs and safe mobility strategies.
- Ensure documentation accuracy and maintain compliance with facility policies and safety standards.
Requirements
- Active physical therapy license to practice in the United States.
- Experience in or knowledge of skilled nursing facility or long-term care settings preferred.
- Ability to work 40 hours per week on a daytime schedule (08:00–16:30) and manage a caseload effectively.
- Strong communication and teamwork skills to coordinate with the care team.
- Willingness to work in a SNF environment for a 13-week travel assignment.
- Ability to pass background check and drug screen as part of the onboarding process.
