Responsibilities
- Conduct initial and ongoing physical therapy evaluations for residents

- Monitor resident progress, document outcomes, and adjust care plans accordingly
- Collaborate with an interdisciplinary team, participate in care planning, and assist with discharge planning
- Ensure patient safety and maintain accurate records in accordance with facility policies
Requirements
- Active physical therapy license to practice in Pennsylvania (or eligible)
- Experience or familiarity with geriatric or skilled nursing facility populations preferred
- Strong assessment, documentation, and communication skills
- Ability to work independently within a team-focused environment and adhere to the assigned shift
- Ability to meet credentialing and compliance requirements for a travel assignment
