Responsibilities
- Provide direct ICU-level nursing care to patients receiving home hospice services, including frequent assessments, monitoring, symptom control, and safety management.
- Administer prescribed medications and IV therapies, monitor patient response, and adjust care plans as indicated.
- Manage complex end-of-life symptoms with a focus on comfort, collaborating with patients, families, and the care team to align with goals of care.
- Document assessments, interventions, and patient outcomes in the electronic medical record; ensure accurate handoffs between team members.
- Coordinate with the interdisciplinary team (physicians, case managers, social workers) to support the patient and family throughout the course of care.
- Educate patients and families on symptom management, medication administration, safety considerations, and care planning.
Requirements
- Active Registered Nurse license in California
- ICU nursing experience or demonstrated ICU competencies
- Ability to work in a home healthcare - hospice setting
- Availability for night shifts (12-hour blocks) as part of the schedule
- Must be able to meet assignment-specific credentialing and compliance requirements as applicable
