Responsibilities
- Conduct comprehensive occupational therapy evaluations for residents within an acute care–informed SNF setting.
- Develop and implement individualized treatment plans focused on activities of daily living and functional independence.
- Deliver direct OT interventions using evidence-based approaches; adjust plans based on progress.
- Document assessments, treatment plans, progress notes, and discharge summaries in a timely, compliant manner.
- Collaborate with the interdisciplinary team (nursing, PT, SLP, social work) to coordinate care and planning for safe transitions.
- Educate residents, families, and caregivers on home exercise programs, safety strategies, and adaptive equipment as appropriate.
Requirements
- Active Occupational Therapist license in Washington state
- Master’s degree in Occupational Therapy from an accredited program
- Minimum of 1 year of clinical experience in acute care or skilled nursing settings preferred
- Ability to work in a skilled nursing facility environment; willing to participate in travel assignment as scheduled
- Current CPR/BLS certification or ability to obtain
