Responsibilities
- Lead coordination of patient care with the interdisciplinary team, ensuring the care plan aligns with the patient’s clinical course and covered services; monitor testing, treatments and procedures for timeliness and appropriateness.
- Act as a liaison among the patient, family, nursing staff, physicians and the broader care team to optimize outcomes and facilitate a smooth discharge plan.
- Manage a defined population’s continuum of care from pre-admission through post-discharge; coordinate transfers to tertiary centers and ensure required information is communicated for ongoing treatment.
- Provide oversight and data collection for regulatory and accreditation compliance; ensure documentation supports program needs.
- Focus on complex patients, including frequent ED utilizers, chronic pain, substance use disorders and homeless patients; collaborate with IP case managers to ensure appropriate resource utilization and length of stay.
- Discuss with physicians the resource utilization, consultations, treatment plan and estimated length of stay; lead efforts to optimize the discharge plan and transitions of care.
- Maintain an efficient, high-volume caseload and participate in relevant internal initiatives or strike teams as needed.
Requirements
- Active Registered Nurse license in California (state of practice)
- RN Case Management certification is required
- Basic Life Support (BLS) certification required
- Minimum 2 years of recent RN experience in case management or a hospital-based care management role
- BSN preferred
- Inpatient/acute care case management experience preferred; ability to manage a heavy workload
- IP CM experience preferred; ability to work effectively in a fast-paced team; no on-call required
