Responsibilities
- Lead the coordination of patient care with multiple disciplines, ensuring the plan aligns with the patient’s clinical course and ongoing needs.
- Serve as a liaison among the patient, family, nurse, physicians, multidisciplinary team, and care benefactors to optimize outcomes.
- Coordinate a continuum of care from pre-admission through post-discharge, ensuring efficient progression through hospitalization.
- Discuss resource utilization, consultations, treatment plans, length of stay estimates, and discharge planning with physicians.
- Focus on complex patients, frequent ED utilizers, chronic pain, substance use, and housing-insecure individuals; collaborate with other case managers to ensure appropriate ongoing care and transfers when needed.
- Ensure regulatory and accreditation data collection and reporting; support transfer of information to tertiary centers as appropriate.
- Perform additional job-related duties as assigned, contributing to a high standard of patient-centered case management.
Requirements
- Active Registered Nurse license in the state of practice (California) or as required by the assignment
- BSN degree preferred
- Case management certification preferred
- Basic Life Support (BLS) certification required
- Minimum of two years of recent experience in the field; inpatient case management experience preferred
- Ability to work in a fast-paced hospital setting and manage complex care plans
- No on-call requirement for this assignment
