Responsibilities
- Provide direct patient care in a skilled nursing setting under supervision of RNs and physicians.
- Administer prescribed medications and document administration accurately.
- Monitor patient status, vital signs, and changes; report findings to supervising clinicians.
- Assist with activities of daily living, including mobility and personal care, as needed.
- Perform basic wound care, dressing changes, and specimen collection as directed.
- Maintain accurate charting, care plans, and documentation in the electronic health record; uphold safety and infection control standards.
Requirements
- Active, unrestricted LPN license in New Mexico.
- Graduation from an approved practical nursing program.
- Ability to work 12-hour day shifts on a 3x12 schedule.
- Prior experience in a skilled nursing or long-term care setting preferred.
- Strong communication and documentation skills; ability to follow care plans and report changes promptly.
