In this LPN Long-Term Care role, you’ll deliver high-quality nursing care tailored to long-term residents with chronic illnesses, dementia, and mobility limitations. Your key responsibilities will include administering medications accurately and on schedule, monitoring vital signs, developing and updating care plans in collaboration with registered nurses and families, and providing wound care, mobility assistance, and ADL support. You’ll play a vital role in infection control, falls prevention, and ensuring comfort and safety around the clock. Documentation and charting will be meticulous, reflecting best practices and regulatory standards. You’ll collaborate with interdisciplinary teams to identify evolving needs, adjust care strategies, and advocate for residents’ preferences and goals.
