Responsibilities
- Manage an assigned patient caseload as the primary RN, coordinating care within the interdisciplinary team.
- Assess patient needs, symptoms, and goals of care; develop and implement individualized care plans.
- Educate patients, families, and caregivers about treatment options, medications, and home care plans.
- Facilitate communication among patients, families, and the care team to ensure clear understanding and adherence.
- Monitor outcomes, adjust plans as needed, and document clinical data in the medical record.
- Coordinate and optimize resources and referrals to support appropriate, cost-effective care outside the acute setting.
Requirements
- Current Registered Nurse license in Colorado (or eligibility) and ability to practice in an outpatient setting.
- Relevant case management or care coordination experience; hospice/home health background is a plus.
- Strong assessment, critical thinking, and clinical decision-making skills.
- Excellent communication and collaboration with an interdisciplinary team.
- Ability to work in a 5x8 weekday schedule with occasional weekend backup or on-call as needed.
