Responsibilities
- Conduct comprehensive OT evaluations for adult residents and develop individualized treatment plans targeting ADLs, mobility, upper-extremity function, and functional independence.
- Implement and adjust therapy interventions to improve daily living activities, safety, and overall quality of life within the skilled nursing setting.
- Monitor progress, document outcomes in the electronic health record, and collaborate with the interdisciplinary team to update goals and plan of care.
- Provide direct patient care during scheduled sessions and participate in discharge planning and care transitions when appropriate.
- Educate patients and families on home exercise programs, adaptive strategies, and safety measures to support continued progress after formal therapy ends.

- Ability to work effectively in a skilled nursing setting
- Strong communication and documentation skills for interdisciplinary collaboration
- Willingness to travel and commit to a 13-week assignment
- Demonstrated clinical reasoning and patient-centered care approach
