Physical Therapist – Home Health travel position in Baker, Montana, United States, starting 06/22/2026 for a 13-week assignment. Responsibilities
- Perform initial evaluations and develop individualized physical therapy treatment plans
- Implement interventions including therapeutic exercise, gait and balance training, manual therapy, and functional mobility activities
- Reassess patient progress and adjust plans to optimize functional goals
- Educate patients and families on home exercise programs, safety, and self-management strategies
- Document assessments, plans of care, progress notes, and communicate with the interdisciplinary care team
- Collaborate with other health professionals and ancillary staff to coordinate patient care and discharge planning
- Maintain professional standards and ensure patient safety and comfort throughout treatment
