Responsibilities
- Conduct psychosocial assessments, abuse/neglect screening, SDOH evaluation, discharge planning, and care coordination; collaborate with interdisciplinary teams to support patient and family needs.
- Provide onboarding and competency development for new social work staff; deliver individual and group education on documentation standards, assessments, interventions, family engagement, and interdisciplinary collaboration.
- Perform chart audits, provide documentation feedback, and support quality improvement initiatives to enhance care processes and outcomes.
- Deliver clinical supervision, coaching, mentoring, and consultation to LCSWs and other social work staff; assess staff development needs and develop training resources.
- Serve as a subject matter expert to support implementation of practice changes, new programs, and departmental initiatives; partner with leadership to promote consistent standards of practice and staff growth.
- Be willing to float to multiple units as needed and participate in on-call coverage, including weekends and holidays when required.
Requirements
- Active MA LICSW license; must have at time of submission.
- 5–10 years of medical social work experience, including psychosocial assessments, SDOH evaluation, abuse/neglect assessments, discharge planning, care coordination, and interdisciplinary collaboration.
- Acute care experience required; experience providing clinical supervision, staff support/coaching, documentation review, and clinical consultation preferred.
- Ability and willingness to float to multiple units as needed; on-call availability (weekends/holidays if on-call).
