Responsibilities
- Deliver direct, patient-centered nursing care to adult residents in a long-term care setting, within the scope of practice for an LPN.
- Monitor residents’ conditions, document changes, and report to the supervising RN or PCP as needed; administer prescribed medications and follow care plans.
- Assist with activities of daily living, mobility, hygiene, nutrition, and safety to support resident quality of life.
- Perform basic treatments and procedures per plan of care; monitor for adverse effects and report concerns promptly.
- Maintain accurate charting and documentation; participate in shift handoffs and care coordination with the interdisciplinary team.
- Uphold infection control practices, patient safety standards, and confidentiality in accordance with facility policies.
Requirements
- Active North Carolina LPN license
- Minimum 12 months of recent LPN experience
- Ability to work evenings in a long-term care setting
- Weekend schedule: Every other weekend required
- Orientation provided at the start of the assignment
- Proficient in basic charting and medication administration
