Responsibilities
- Conduct initial and ongoing occupational therapy evaluations for residents, focusing on activities of daily living, mobility, cognition, fine motor function, and safety with daily tasks; develop and update individualized treatment plans with measurable goals.
- Implement evidence-based OT interventions, including therapeutic activities, therapeutic exercises, ADL/IADL training, upper-extremity rehabilitation, splinting, and assistive device instruction; modify goals as progress is documented.
- Provide documentation of assessments, treatment plans, progress notes, and discharge planning in alignment with facility policies and payer requirements; maintain high standards of accuracy and timeliness.
- Collaborate with a multidisciplinary team (nursing, physical therapy, speech therapy, social work, and physicians) to coordinate care plans, monitor progress, and facilitate safe transitions to next levels of care or home.
- Educate residents and family members on home exercise programs, adaptive strategies, safety precautions, fall prevention, and device management to support continued progress after discharge.
- Ensure compliance with regulatory standards, infection control practices, patient privacy (HIPAA), and facility policies; participate in regular care conferences and quality improvement activities.

Requirements
- Demonstrated experience providing occupational therapy services in acute care or skilled nursing/post-acute settings
- Ability to work in a travel assignment with a two-week contract and adapt to a new unit as needed
- Strong documentation, communication, and collaboration skills for interdisciplinary care
- Ability to work onsite in Monterey, California and adhere to a 5x8 day shift
- Eligible to work in the United States and legally able to perform required duties
