Travel Physical Therapist Job
Physical Therapist position in Reading, Pennsylvania, United States. Travel assignment starting 04/20/2026 for 13 weeks in a skilled nursing facility. Responsibilities
- Conduct initial PT evaluations and develop individualized treatment plans for residents
- Implement therapy interventions to improve mobility, strength, balance, gait, and functional independence
- Document assessment results, treatment plans, progress notes, and discharge summaries in the medical record
- Reassess patient status, monitor progress, and adjust treatment plans as needed
- Collaborate with an interdisciplinary team (nursing, OT, SLP, case management) to coordinate care
- Educate residents and families on home exercise programs, ADLs, safety strategies, and fall prevention
Requirements
- Active Physical Therapist license in Pennsylvania or eligibility for PA licensure
- Doctor of Physical Therapy (DPT) degree or equivalent
- Ability to work in a skilled nursing facility setting
- Availability to commit to a 13-week travel assignment with a consistent 8:00–16:30 schedule
- Strong documentation, communication, and teamwork skills; proficient in EMR
