Responsibilities
- Plan, organize and direct home care services to meet patients’ needs in their homes; manage a patient caseload effectively.
- Perform initial and ongoing nursing assessments; document patient status, response to treatment, and plan of care.
- Provide direct and/or indirect skilled nursing care within the plan of care; counsel and educate patient and family on nursing and related needs.
- Update nursing plans, medication profiles, and documentation promptly; ensure timely submission of all records per policy.
- Coordinate patient care with other disciplines; incorporate input from the care team and adjust the plan of care as goals are clarified.
- Communicate changes to goals or status with the patient and family, and serve as a resource to patients, families, and other care team members.

Requirements
- Licensure: Registered Nurse (RN) license in the state of practice (California) required
- Certifications: CPR or Basic Life Support (BLS) required
- Experience: At least two years of recent experience in the field
- Education: Bachelor’s Degree in Nursing (BSN) preferred
- Additional requirements: Must have EPIC experience; valid Driver’s License and must be at least 21 years old; current automobile insurance and a reliable automobile; ability to work in a home health environment
