Responsibilities
- Provide registered nurse care within the Diabetes Clinic and Primary Care Clinic, including consultation, assessment, and evaluation of diabetes-related services for individuals, families, and communities.
- Deliver diabetes self-management education, collaborate with the care team, and coordinate follow-up plans and referrals.
- Perform triage and risk assessment using established protocols, with emphasis on Emergency Severity Index (ESI) knowledge.
- Document care in the electronic health record and communicate care plans across the team; ensure patient safety and adherence to clinical guidelines.
- Educate patients on diabetes management, lifestyle modifications, medication adherence, and preventive care; support care transitions and care coordination.

Requirements
- Active Registered Nurse license; BLS and ACLS certified; current Certified Diabetes Educator (CDE) certification obtained within the last 3 years prior to start date
- Clinic experience mandatory, with preference for Diabetes Clinic and Primary Care Clinic exposure
- Minimum of 3 years of recent RN experience in the applicable specialty
- Ability to work in a clinic setting; strong critical thinking and triage skills
- Knowledge of ESI triage and the ability to apply it in clinical scenarios
- Orientation: Every other Monday during onboarding
