Responsibilities
- Conduct initial and ongoing physical therapy evaluations for patients in their homes and develop individualized plans of care.
- Implement evidence-based PT interventions to improve mobility, strength, balance, gait, ROM, and functional independence; adjust plans as goals evolve.
- Perform ongoing re-evaluations, document progress, and discharge patients when goals are met or care is transitioned.
- Complete required documentation, including assessments and progress notes; participate in relevant home health processes and interdisciplinary team communication (e.g., care plans, family education).
- Maintain patient safety and infection control standards; collaborate with physicians, nurses, and other care team members; travel to patient homes within the assigned territory and beyond as needed.
- Manage time and caseload effectively to meet weekly visit expectations, including possible weekend coverage or travel outside the service area when required.

Requirements
- Current California Physical Therapist license (PT)
- BLS certification
- Valid driver's license with auto insurance and vehicle registration
- TB test (annual), MMR, Varicella, HepB, TDap (within 10 years), Flu (annual)
- Ability to pass drug screen and physical examination
- Education: Doctoral/Professional degree in Physical Therapy (e.g., DPT) from an accredited program
- Willingness to travel within and outside the service area and to see patients who may be Covid-positive or have COVID-19 exposure
- Prior home health or OASIS documentation experience preferred but not required
