Responsibilities
- Conduct comprehensive physical therapy evaluations and establish individualized treatment plans for adult residents in a skilled nursing setting.
- Provide evidence-based therapy to improve mobility, strength, balance, gait, and activities of daily living; adjust plans of care based on progress.
- Implement therapeutic interventions (therex, manual therapy, gait training, functional mobility training, assistive device training) and oversee appropriate modalities as prescribed.
- Document assessments, progress notes, goals, and discharge planning clearly and timely; ensure continuity of care across shifts and disciplines.
- Collaborate with an interdisciplinary team, participate in care conferences, and educate residents and families about expectations, safety, and home or facility-based strategies.
- Maintain safety, infection control, and compliance with clinical protocols and state practice acts.
Requirements
- Active Physical Therapist license in Maryland (or eligible to practice in Maryland)
- Graduation from an accredited physical therapy program
- Experience in a skilled nursing facility or geriatric rehabilitation setting preferred
- Strong communication, documentation, and collaboration skills
- Ability to work effectively in a multidisciplinary team and adhere to the established plan of care
