Responsibilities
- Provide direct patient care to residents under supervision of an RN, including routine assessments and observations
- Administer medications and document administration accurately per policy and state regulations
- Monitor vital signs, respond to changes in patient condition, and report promptly to the supervising clinician
- Assist residents with activities of daily living and mobility, promoting safety and dignity
- Document care provided, update care plans, and participate in routine handoffs and interdisciplinary rounds
- Support care coordination with ancillary services and family members as appropriate
Requirements
- Active Licensed Practical Nurse license in New Mexico
- Completion of an approved practical nursing program
- Prior experience in a long-term care or skilled nursing setting preferred
- Ability to work a 36-hour weekly schedule across 3x12-hour shifts
- Proficiency with basic electronic health records and documentation
- Excellent communication and teamwork skills to support resident-centered care
