Responsibilities
- Conduct comprehensive physical therapy evaluations for residents in a long-term care setting.
- Develop, implement, and adjust individualized treatment plans to meet established goals.
- Provide therapeutic exercise programs, gait and balance training, manual therapy as appropriate, and functional mobility interventions.
- Document patient progress, modify treatment plans based on clinical findings, and communicate with the care team.
- Ensure patient and family education on therapy goals, home programs, safety, and fall prevention.
- Maintain compliance with regulatory requirements and facility policies; participate in care conferences and discharge planning as needed.
Requirements
- Active physical therapy license in New Mexico or current eligibility for licensure in New Mexico.
- Ability to work effectively in a multidisciplinary team within a long-term care setting.
- Strong documentation, assessment, and communication skills.
- Willingness to work a travel assignment in Silver City, NM; reliable transportation as needed.
- Experience with geriatric populations and mobility-focused rehabilitation is preferred.
