Responsibilities
- Conduct initial physical therapy evaluations and establish individualized treatment plans for residents in an acute rehab/long-term care setting.
- Deliver therapeutic exercise programs, gait and balance training, manual therapy, modalities, and functional mobility interventions as appropriate.
- Monitor patient progress, adjust treatment plans, and document outcomes in the chart/EHR according to facility policy.
- Collaborate with an interdisciplinary team (nursing, physicians, therapy disciplines) to support discharge planning and goal attainment.
- Educate patients and families on home exercise programs, safety, and progression, reinforcing best practices for functional independence.
- Ensure patient safety, adherence to clinical guidelines, and compliance with infection control and regulatory standards.
Requirements
- Active PT license in Texas or eligibility for Texas licensure (verification of licensure will occur during hiring)
- New graduates welcome
- Ability to work 8-hour shifts, Monday through Friday (08:00–17:00)
- Capacity to work in a LTC – Acute Rehab setting with an interdisciplinary team
- Eligible for a travel assignment and able to relocate to the Dallas area
