Responsibilities
- Perform OT evaluations, develop treatment plans, and implement therapy interventions for residents, applying acute care principles in a long-term care context.
- Provide individual and group therapy activities focused on activities of daily living, mobility, balance, endurance, transfers, and safety.
- Monitor patient progress, document outcomes, adjust treatment plans as needed, and communicate findings to the interdisciplinary team.
- Ensure patient safety and maintain adherence to care plans, infection control, and facility policies during interventions.
- Collaborate with occupational therapy supervisors, nursing, physical therapy, social work, and discharge planning to support seamless transitions and goals.
- Educate patients and families on home exercises, activity modification, and strategies to maintain function after discharge.

Requirements
- Active Certified Occupational Therapy Assistant license in New Hampshire
- Graduation from an accredited OTA program
- Experience in acute care or inpatient rehabilitation settings; experience in long-term care is considered relevant
- Ability to work in a fast-paced, high-need care environment within a long-term care setting
- Availability to start on 08/11/2026 and commit to the 10-week assignment
