Responsibilities
- Deliver coordinated nursing care to hospice patients in home settings, applying the nursing process from assessment through evaluation.
- Monitor patient status, administer medications, manage symptoms, and respond to changes in condition; perform CPR/BLS as required.
- Collaborate with the interdisciplinary team and family to plan, implement, and evaluate care; participate in discharge planning and care transitions.
- Supervise and delegate duties to assigned nursing staff and aides, ensuring safe, compliant, and compassionate care.
- Maintain accurate documentation in the electronic health record (EPIC) and ensure patient privacy and safety.
Requirements
- Active Registered Nurse license in the state of practice (California) required
- Current Basic Life Support (BLS) or CPR certification required
- ACLS certification required
- Bachelor’s Degree in Nursing (BSN) preferred
- Minimum 2 years of recent RN experience in the field
- EPIC experience required
- Ability to work night shifts and travel as part of a travel assignment
