Responsibilities
- Support the supervising physical therapist in implementing the plan of care for LTAC residents.
- Provide therapeutic exercises, gait training, range-of-motion activities, strengthening, and balance interventions as directed.
- Monitor patient responses during therapy, document progress, and communicate changes to the care team.
- Assist with transfers, positioning, and activities of daily living within the LTAC/SNF setting.
- Maintain safety and infection control standards during therapy sessions and ensure equipment is used properly.
- Collaborate with the interdisciplinary team to coordinate documentation, discharge planning, and rehabilitation goals.
Requirements
- Ability to work in a LTAC/Skilled Nursing Facility setting.
- Willingness to commit to a 17-week travel assignment.
- Availability to work 35 guaranteed hours per week.
- Eligibility to work in California and ability to meet facility policies and procedures.
- Strong communication skills and ability to work within a multidisciplinary team.
