Responsibilities
- Deliver coordinated nursing care for an individual patient or assigned group per established standards, the nursing process, and patient needs
- Supervise and direct activities of licensed and unlicensed nursing staff; coordinate care with other disciplines
- Analyze assessment data, identify diagnoses and care issues, and develop and implement a plan to achieve defined outcomes
- Monitor patient progress, evaluate outcomes, and adjust care plans as needed
- Provide patient and family education, and participate in discharge planning to ensure continuity of care
- Delegate appropriately and coordinate duties of the care team; maintain a safe, healing environment
Requirements
- Current Registered Nurse (RN) license in the state of practice
- BLS/CPR certification from an approved vendor
- ACLS, PALS, and NIH Stroke Scale (NIHSS) certifications from approved vendors
- EPIC electronic medical record experience within the last 12 months
- At least two years of recent nursing experience in a relevant field
- BSN preferred
- Ability to work in a home healthcare - hospice setting; night shift availability required
- Must have current nursing license and be able to practice per state requirements
