Responsibilities
- Administer prescribed medications and treatments per orders; document in the chart accurately.
- Monitor resident conditions, perform ongoing assessments, and report changes promptly to the supervising clinician.
- Provide routine nursing care and support activities of daily living, including mobility, hygiene, and comfort measures.
- Perform wound care and support skin integrity; assist with ostomy care as appropriate within scope.
- Maintain clear, timely documentation; ensure compliance with regulatory requirements and facility policies.
- Collaborate with the interdisciplinary team; participate in care planning, communication with residents and families as appropriate, and effective shift handoffs.
Requirements
- Active LPN license in Pennsylvania
- Graduate of an accredited practical nursing program
- Current CPR/BLS certification
- Minimum 1 year of licensed practical nurse experience; long-term care or geriatric experience preferred
- Ability to work evenings and every other weekend as scheduled
- Strong communication and documentation skills; ability to work effectively in a multi-disciplinary LTC team
