Responsibilities
- Provide direct nursing care to residents under supervision of an RN or facility leadership
- Monitor vital signs, administer medications and treatments per the care plan and facility policy
- Assist residents with activities of daily living, mobility, hygiene, nutrition, and comfort
- Perform basic wound care, dressing changes, and other routine procedures as authorized
- Document care accurately in the charting system and communicate changes promptly to the supervising team
- Maintain safety, infection control, and patient safety standards; respond to call lights and emergencies as needed
Requirements
- Active, unrestricted LPN license in the appropriate state
- Basic Life Support (BLS) certification
- Completion of an approved practical nursing program
- Prior experience in a long-term care or geriatric setting preferred
- Ability to work 12-hour shifts and adapt to a fast-paced LTC environment
