Responsibilities
- Administer prescribed medications and treatments per physician orders and established clinical protocols
- Monitor residents, perform routine assessments, document vital signs, and report changes promptly
- Provide wound care and other basic nursing procedures appropriate for an LPN in a long-term care setting
- Assist with activities of daily living, mobility, and transfer support to promote safety and independence
- Document care plans and progress notes in the electronic health record; communicate changes to RN and care team
- Collaborate with the interdisciplinary team to ensure safe, high-quality resident care, while adhering to infection control and safety standards
Requirements
- Active LPN license in good standing
- Ability to work a 3x12 night shift and rotating schedule (EOW)
- Prior experience in a long-term care or geriatric nursing environment preferred
- Strong documentation, assessment, and communication skills
- Ability to pass background check and any required screenings; comply with standard assignment protocols
