Responsibilities
- Assess residents’ PT needs, perform initial and periodic evaluations, and develop evidence-based, individualized treatment plans aligned with goals and regulatory requirements.
- Implement PT interventions such as gait training, therapeutic exercises, balance and coordination activities, functional mobility, and wheelchair/orthotic management; monitor response and adjust plans accordingly.
- Document evaluations, progress notes, discharge summaries, and outcomes using standard formats; ensure accuracy and compliance with facility policies and payer requirements.
- Collaborate with the interdisciplinary team (nursing, OT, speech, therapy managers) to adjust goals, coordinate care, and ensure smooth transitions across services.
- Educate patients and families on home exercise programs, safety strategies, fall prevention, and post-discharge instructions; provide written plans and ensure understanding.
- Maintain clinical areas and equipment, uphold infection control practices, and protect patient safety and privacy in all therapy activities.
Requirements
- Doctor of Physical Therapy (DPT) degree from an accredited program
- Active California Physical Therapy license in good standing with the ability to verify licensure
- Current Basic Life Support (BLS) or CPR certification
- Demonstrated ability to work in a skilled nursing facility setting with geriatric populations
- Willingness to travel and commit to a 13-week assignment; ability to adapt to daytime shifts and variable patient schedules
