Responsibilities
- Perform comprehensive speech-language pathology evaluations for residents, including communication and swallowing assessments
- Develop and implement individualized treatment plans targeting speech, language, cognitive-communication, and swallowing goals
- Provide direct, one-on-one therapy and group therapy as appropriate; monitor progress and adjust treatment plans accordingly
- Document resident care thoroughly in the medical record and communicate progress with the care team
- Collaborate with interdisciplinary staff to support discharge planning and resident goals; participate in team meetings as needed
Requirements
- Active licensure to practice as a Speech Language Pathologist (state requirements apply)
- Ability to provide clinical services in a skilled nursing facility setting
- Strong assessment, treatment planning, and documentation skills
- Effective communication and teamwork abilities; comfortable interfacing with residents and families
- Eligibility for travel assignments and adherence to assignment schedule
