Responsibilities
- Coordinate incoming office referrals and verify insurance coverage
- Manage incoming calls from patients, families, physicians, and care team disciplines
- Ensure that hospitalizations are communicated and follow up with patients admitted to other facilities
- Coordinate small programs with limited budgets and serve as a working supervisor for a team or work group
- Perform as a clinical intake coordinator for home health processing and scheduling referrals; no on-call responsibilities
Requirements
- Active LVN/LPN license (California) with LVN certification
- Minimum 1 year of relevant experience, preferably in clinical intake coordination or home health processing
- Ability to work the day-shift schedule (M-F, 8:00 a.m.–5:00 p.m.) in a home health/hospice environment
- Excellent communication, coordination, and problem-solving skills
