Responsibilities
- Administer medications and document administration accurately in resident records
- Monitor residents’ health status, perform assessments, and report changes to the care team
- Perform routine nursing tasks (vital signs, wound care, and other duties as permitted)
- Respond to resident calls, manage care plans, and coordinate with the interdisciplinary team
- Maintain infection control practices and safety standards
- Document care provided and ensure regulatory compliance
Requirements
- Active LPN license in good standing
- CPR/BLS certification
- Ability to work flexible shifts and meet the guaranteed 40-hour weekly schedule
- Excellent communication and teamwork skills
- Prior experience in long-term care or skilled nursing is preferred
