Responsibilities
- Perform initial and ongoing physical therapy evaluations of residents to determine functional status and rehab goals.
- Develop and implement individualized treatment plans focusing on mobility, strength, balance, gait, and activities of daily living.
- Administer therapeutic interventions such as therapeutic exercise, manual therapy, gait training, and functional training to promote patient outcomes.
- Document patient progress and treatment plans in the electronic health record; communicate updates to the care team and adjust plans as needed.
- Collaborate with physicians, nursing staff, and other therapists to coordinate care, participate in care conferences, and assist with discharge planning and caregiver education.
- Ensure patient safety and adherence to applicable regulatory and facility standards, including infection control and privacy requirements.
Requirements
- Current license to practice physical therapy in the United States (licensure status to be verified).
- Experience or demonstrated interest in long-term care or skilled nursing settings preferred.
- Ability to work a 13-week assignment with guaranteed 35 hours per week on a daytime schedule.
- Strong communication and documentation skills; ability to work within an interdisciplinary team.
- Ability to meet assignment requirements and work in a long-term care setting.
