Responsibilities
- Administer prescribed medications and document administration in accordance with policy and procedures.
- Monitor residents’ conditions, perform routine assessments, record vital signs, and report changes to the supervising clinician.
- Assist with activities of daily living (ADLs) and provide comfort measures to residents as needed.
- Perform basic nursing procedures under supervision, and support the care planning process with accurate documentation.
- Collaborate with RNs, physicians, and the interdisciplinary team to ensure patient-centered care and safe, compliant practices.
- Maintain infection control standards, safety protocols, and incident reporting; respond promptly to call bells and emergencies.
Requirements
- Active LPN license in the United States (VT license preferred)
- Graduation from an approved practical nursing program
- BLS/CPR certification (per American Heart Association or equivalent)
- 1+ year of recent experience in long-term care or skilled nursing settings preferred
- Ability to work 40 hours per week with day and evening shifts; strong communication and documentation skills
