Responsibilities
- Conduct comprehensive occupational therapy evaluations for residents in a geriatric skilled nursing setting.
- Develop and implement individualized treatment plans to improve or maintain activities of daily living, mobility, and functional independence.
- Administer therapeutic activities and interventions focusing on fine motor skills, upper-extremity function, balance, cognition, and safety.
- Reassess and modify treatment plans based on progress, goals, and regulatory guidelines; document progress clearly in the chart.
- Collaborate with an interdisciplinary team including physical therapy, speech-language pathology, nursing, and social work to optimize resident outcomes.
- Provide education to residents and families on therapy goals, home exercise programs, safety, and transition planning.

Requirements
- Active Occupational Therapist license in California or eligibility to obtain licensure.
- Degree from an accredited occupational therapy program (bachelor’s or master’s; certification preferred per local requirements).
- Experience in geriatric rehabilitation or skilled nursing facility settings is preferred.
- Strong documentation, communication, and interpersonal skills; ability to work in a fast-paced, multi-disciplinary environment.
- Basic knowledge of current occupational therapy standards of practice and regulatory requirements; ability to use applicable documentation systems.
- BLS certification preferred; ability to meet facility credentialing and reference checks.
