Responsibilities
- Administer prescribed medications and treatments according to physician orders and facility policy
- Monitor resident conditions, record vital signs, and promptly report changes to the supervising RN
- Perform skilled nursing duties such as wound care, injections, and assistance with activities of daily living
- Assist with routine assessments and care planning; document care accurately in the electronic health record
- Maintain infection control practices, patient safety, and OSHA/facility standards
- Collaborate with RNs, physicians, and the interdisciplinary team to coordinate resident care
Requirements
- Active Licensed Practical Nurse (LPN) license in New Hampshire
- Current CPR/BLS certification
- Prior experience in long-term care or skilled nursing settings preferred
- Ability to work the specified evening shift and weekend schedule
- Strong communication and teamwork skills; reliable and able to follow detailed care plans
- Eligible to work in the local area and pass standard employment screenings
