Responsibilities
- Provide direct nursing care to residents in a memory care unit, including routine assessments and documentation.
- Administer medications and treatments per physician orders; monitor for adverse reactions and effectiveness.
- Monitor residents’ conditions, respond to changes, and coordinate care with the interdisciplinary team.
- Support activities of daily living, mobility, and comfort measures; assist with transfers and fall prevention.
- Conduct basic wound care, monitor treatment plans, and update care plans as needed.
- Ensure safety, privacy, and respect for residents; maintain accurate charts and compliance with regulatory requirements.
Requirements
- Active LPN license in Vermont
- Diploma or certificate from an accredited practical nursing program
- Ability to work in a memory care setting and collaborate with a multidisciplinary team
- Experience in long-term care or memory care settings preferred (not required)
- CPR/BLS certification preferred
