Responsibilities
- Conduct initial and ongoing speech-language pathology evaluations for residents, establishing goals and treatment plans aligned with acute care and SNF needs.
- Provide therapy for communication and swallowing disorders, including dysphagia management, with progression notes and adjustments to serve patient safety and function.
- Implement, document, and monitor individualized treatment plans; deliver bedside therapy and caregiver/staff education to support carryover.
- Collaborate with an interdisciplinary team to coordinate discharge planning, transitions of care, and patient/family education.
- Maintain timely, accurate documentation in the facility’s medical record, adhere to regulatory and facility policies, and participate in care conferences and quality improvement activities.
- Participate in performance improvement and safety initiatives as appropriate.
Requirements
- Active Maryland state license for Speech-Language Pathology (SLP)
- CCC-SLP certification or eligibility for certification
- Master’s degree in Speech-Language Pathology
- Experience in acute care or skilled nursing facility settings preferred
- Ability to work effectively within an interdisciplinary team and in a skilled nursing environment
- Strong communication and documentation skills; compliance with credentialing and facility requirements
