As part of a dedicated geriatric care team, you will assess, plan, implement, and evaluate care that supports independence, safety, and quality of life for residents with chronic conditions, cognitive changes, and end-of-life considerations. In this Long-Term Care LPN role, you will perform comprehensive resident assessments, administer medications, monitor vital signs, manage wound care, assist with activities of daily living, develop and update care plans in collaboration with RNs and physicians, ensure infection control, and participate in discharge planning and family communication. You’ll support supervising nurses, document care clearly in electronic health records, and contribute to quality improvement initiatives. The position offers a blend of stable hours and flexible extensions—your contract can be extended based on performance and facility needs.
