Responsibilities
- Conduct initial and ongoing PT evaluations for residents, developing and updating individualized treatment plans to address mobility, strength, balance, gait, ROM, and functional activities.
- Deliver evidence-based therapeutic interventions aimed at improving independence with activities of daily living and overall functional status; tailor programs to cognitive or medical complexities.
- Reassess patient progress regularly, document SOAP notes, and communicate changes in the plan of care with the interdisciplinary team.
- Educate residents and families on home exercise programs, safety strategies, and long-term goals; facilitate appropriate discharge planning and referrals as needed.
- Collaborate with nursing, therapy assistants, and other clinicians; participate in care conferences and contribute to care planning and goal setting.
- Maintain compliant documentation, adhere to infection control and safety protocols, and promote patient safety during all interventions.
Requirements
- Active Physical Therapist license in New Mexico, or eligibility for NM licensure
- Experience or demonstrated interest in geriatric or skilled nursing facility settings
- Strong documentation and communication skills; ability to collaborate with an interdisciplinary team
- Availability to work the 08:00–16:30 shift and participate in a 13-week travel assignment
- Proficiency with standard therapy modalities and patient safety procedures in a long-term care environment
