Responsibilities
- Conduct comprehensive speech-language evaluations and formal assessments for residents with communication and/or swallowing concerns.
- Develop, implement, and monitor individualized treatment plans aligned with PDPM requirements and best practice standards.
- Provide direct therapy to residents; track progress and adjust goals based on ongoing assessment.
- Document all evaluations, treatment sessions, and outcomes in the electronic medical record (Epic EMR); ensure documentation is accurate and timely.
- Collaborate with interdisciplinary team members (nursing, rehabilitation, social work) to optimize communication and swallowing outcomes; participate in care planning meetings.
- Educate residents, families, and staff on communication strategies, compensatory techniques, and safe swallowing practices; promote patient-centered care.
- Ensure compliance with regulatory guidelines, facility policies, and professional standards; maintain patient safety, privacy, and confidentiality.
- Monitor and report changes in patient status; communicate recommendations to the care team promptly.

Requirements
- Current New York State license as Speech-Language Pathologist
- Eligible for or holds the Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from ASHA
- Graduate of an ASHA-accredited program with a degree in Speech-Language Pathology
- LTC experience required
- Proficient with Epic EMR and MS Office; able to document accurately and timely
- Ability to work effectively in a skilled nursing facility setting and to collaborate within a multidisciplinary team
