Social Worker (Medical Social Worker) – Everett, WA. Travel - temporary assignment starting May 18, 2026 for 13 weeks. Weekly pay range: $1,321–$1,365 per week. Guaranteed hours: 72 hours per 2-week period. Shift: 4x8 days, days. Setting: hospital with a Level II Trauma Center across two campus locations in close proximity.

Responsibilities
- Provide medical social work services, including discharge planning, care coordination, and patient/family counseling for adults and geriatrics in an acute care setting.
- Assess psychosocial needs, develop care plans, and coordinate with an interdisciplinary team (PT, interpretation services, Social Services, Pharmacy, Transportation) to support safe, timely transitions.
- Facilitate transitions to home health, rehabilitation, or long-term care, including referrals and follow-up planning.
- Document assessments, interventions, and care plans in EPIC with timely charting and compliance.
- Float across hospital areas and both campus locations as needed to meet patient care needs; participate in holiday and weekend coverage as required.
Requirements
- Active WA social work license (LICSW, LSWAIC, LASW, or LSWAA); pending license accepted with client confirmation; confirm status prior to submittal.
- Minimum 1 year clinical experience; acute care experience with discharge planning/care coordination required.
- Preferred: diverse case management experience (Med-Surg, prolonged LOS, ED).
- BLS certification; EPIC experience; ability to work in an acute care setting and participate in cross-unit floating.
- Eligibility to work in Everett, WA; ability to comply with facility’s on-site requirements.
