Responsibilities
- Conduct acute care social work assessments for adult and geriatric patients; develop and monitor care plans and discharge plans
- Coordinate discharge planning and post-discharge resources to support safe transitions
- Collaborate with interdisciplinary teams (PT, Interpretation Services, Social Services, Pharmacy, Transportation) to coordinate services and referrals
- Maintain a caseload typically 18–24 patients, with higher volumes on weekends
- Document assessments, plans, and progress in EPIC; participate in rounds and care coordination meetings
- Float across units and both campuses as needed; adapt to varying patient acuity and care needs

Requirements
- Washington state social work licensure required; LMSW or LCSW accepted for social work roles; verify license status with hiring team; pending-license submittal acceptance may be considered
- BLS certification required
- Education: Master’s degree in Social Work (MSW) or equivalent; LMSW/LCSW acceptable per WA licensure guidance
- Experience: Minimum 1 year of relevant experience; acute care, discharge planning, and care coordination preferred; first-timers accepted
- Charting: EPIC experience required
- Additional: Ability to work across multiple units and the two campus locations; no local candidates accepted (50-mile radius rule)
- Compliance: COVID-19 vaccine not required
- Orientation and pre-start: 40 hours of orientation; pre-start training modules (~16 hours on average; >20 hours requires approval)
- RTO: All approved time off to be requested at submittal; up to 5 days allowed
- References: 2 references within the last 3 years
