Responsibilities
- Administer medications and treatments per physician orders in accordance with facility policy and regulatory guidelines.
- Monitor patients, perform ongoing assessments, document vital signs, and report changes to the care team.
- Collaborate with the interdisciplinary team to develop, implement, and adjust care plans; participate in discharge planning as appropriate.
- Provide direct patient care including assistance with activities of daily living, safety measures, and comfort interventions; respond to call bells and emergencies promptly.
- Document care and outcomes accurately in the electronic health record and maintain clear communication with supervisors, families, and team members.
Requirements
- Active Colorado Registered Nurse license
- CPR/BLS certification (or ability to obtain prior to start)
- Experience in skilled nursing facility or long-term care setting preferred
- Ability to work night shift (23:00–07:00) and communicate effectively with patients and team
- Strong clinical assessment, documentation, and collaboration skills
- Ability to pass background check and drug screen
