- Conduct initial and ongoing occupational therapy assessments for residents in the assigned facilities

- Document evaluations, treatment plans, progress notes, and discharge summaries in the electronic health record
- Collaborate with an interdisciplinary team to optimize care planning, goals, and referrals
- Ensure patient and staff safety, adhere to infection control practices, and provide education to patients and families
- Participate in care conferences and coordinate discharge planning as needed
Requirements
- Active Occupational Therapist license in Washington state or eligibility for licensure
- Ability to practice in a long-term care setting and adapt to a multi-site assignment
- Strong documentation and communication skills; proficient with standard EHR systems
- Availability to work guaranteed 35 hours per week for 13 weeks
- Willingness to travel between two facilities as part of a shared assignment
- Education: Master’s degree in Occupational Therapy from an ACOTE-accredited program (or equivalent)
