Responsibilities
- Administer prescribed medications and treatments according to orders, with accurate documentation in the charting system.
- Monitor residents for changes in condition, report findings to the supervising RN, and respond promptly to clinicians’ instructions.
- Provide direct patient care including activities of daily living, mobility assistance, and safety support for residents.
- Observe and document vital signs, intake and output, pain levels, and wound assessments as directed; ensure timely escalation when indicated.
- Respond to call lights; triage basic needs and coordinate with the nursing team to maintain care plans.
- Collaborate with licensed nurses, therapists, and ancillary staff to deliver comprehensive, person-centered care aligned with each resident’s plan of care.
Requirements
- Active Licensed Practical Nurse license in good standing
- Ability to work night shifts (3x12) in a long-term care setting
- Willingness to travel for a temporary assignment
- Proficiency with basic electronic health records and routine clinical documentation
- Able to perform typical patient care tasks and assist residents with mobility and daily activities
- Ability to pass routine background checks and credentialing required for temporary clinical assignments
